Enter your name exactly as it appears on your ID
- Use your full legal name, not a nickname, short form or initials. For example, enter “William”, not “Bill”, if your ID says William.
- Include every middle name shown on your ID, in the same order. If your ID has no middle name, leave that field blank.
- Match the spelling exactly, including hyphens, apostrophes and accents (for example, “Marie-Ève O'Neil”).
- Put each name in the right field: first name, middle name(s) and last name (family name) as they appear on your ID.
- Use your current legal name. If you recently changed your name, use the name on the ID you will use to verify your identity.
Why it matters
The check is processed and the result is issued under the name you enter. Organizations that receive your result usually compare it with your ID. For example, the Government of Ontario requires the name on a Criminal Record and Judicial Matters Check used for a security guard or private investigator licence to match the applicant's identification exactly (ontario.ca).
Refunds
If your check is submitted to our screening provider with a name that does not match your ID, the fee is not refundable and you would need to order a new check. Please review your name carefully on the review page before you pay. See our Refund Policy for details.
