Consent and Authorization

Check type confirmation

I have confirmed with my employer or the requesting organization that the check type I selected (Level 1 or Level 2) is the type of check they require. I understand this is not a Vulnerable Sector Check.

Screening consent

1. Authorization for the screening requested

I, [Applicant name], authorize Ontario Criminal Record Check to request a Criminal Record and Judicial Matters Check (Level 2) or Criminal Record Check (Level 1), as selected about me through its screening provider, Triton Canada, for the following purpose: "[Purpose]". I understand this is a name-based criminal record check. It is not a Vulnerable Sector Check, and it is not a fingerprint-based certified check. I understand the screening provider may be unable to confirm a result using name and date of birth alone, and in that case may require additional steps, such as fingerprint verification, before a result can be issued.

Checkbox: “I authorize this criminal record check for the purpose stated above.”

2. Information shared to process the check

To process this request, Ontario Criminal Record Check will share the personal information I provide in this application (legal name, any previous names, date of birth, contact details and address) with Triton Canada. The provider and the record sources it is authorized to query will use this information only to verify my identity and conduct the check. The provider may collect additional identity information directly from me through its own secure verification process.

Checkbox: “I consent to this information being shared for these purposes.”

3. Confirmation of identity

I confirm that I am the person named in this application, that the information I have provided is true and complete, and that I am not requesting a check about another person. I understand that providing false information may result in the request being cancelled without refund and may be reported where required by law.

Checkbox: “I confirm I am the applicant and my information is accurate.”

4. Privacy and delivery of results

I have read the Privacy Policy. My result will be delivered only to me through my secure account. Ontario Criminal Record Check will not disclose my result to an employer or any other organization, including one that paid for or requested the check, unless I give a separate authorization after I have seen my result, or where disclosure is required by law. I may withdraw this consent before the check is submitted to the provider by contacting support; withdrawal after submission may not stop processing already under way.

5. Electronic signature

I agree that typing my full legal name below and selecting "I agree and sign" is my electronic signature and has the same effect as a handwritten signature on this authorization.

Checkbox: “I agree to sign electronically.”

Result disclosure consent

Your result is delivered only to you, in your secure account. It is never shared with an employer or other organization simply because that organization invited you or paid for the check.

If an organization asks for a copy, you can choose to share it, and only after you have viewed your result. When you do:

you see the organization's name and how long it will have access before you agree;
you give a separate, signed authorization that names your application reference and the organization;
the organization can view or download a copy through its secure business account for the stated period only;
you can revoke the authorization at any time from your dashboard. Revocation does not affect copies already downloaded.

Withdrawing consent

You may withdraw your screening consent before your application is submitted to the provider by contacting support. Withdrawal after submission may not stop processing already under way. See the Refund Policy and Privacy Policy.
